Position: Home Care Program Coordinator
Employment type: Full-Time, Exempt Office
Location: 2 days Chicago Office, 2 days Mt. Prospect
Work Hours: 40 hours a week (1 remote work day/week), M-F 8:30 AM - 4:30 PM
Wage range: $45,000 - $50,000
Supervisor: Home Care Program Manager
OVERVIEW:
Home Care Program Coordinator is responsible for overseeing the day-to-day operations of the home care program, ensuring the quality of care provided to clients and the effective management of Home Care Aides (HCAs). This role also involves coordinating effective communication with the Illinois Department on Aging (IDOA), Care Coordination Units (CCU) and Managed Care Organizations (MCO). The coordinator will work closely with clients, their families and the HCAs to ensure compliance with program standards and deliver quality services.
